As a service to our public and the trade community, U.S. Customs & Border Protection (CBP) has made available electronic forms that can be completed online and then printed. The forms comply with Federal accessibility requirements established under Section 508 of the Disabilities Act.
How to Order Printed Forms
The National Distribution Center (NDC) warehouses in Indianapolis, Indiana. To order forms in bulk from the warehouse, please submit CBP Form 262 found on this page to:

The forms are free, however, please Include your shipping account number to cover delivery costs (i.e., Federal Express, DHL, etc.).

Most orders are processed within a 24-hour period.
Individual Serially Numbered Forms
Serially numbered forms are not available electronically (e.g., CBP 5129, Crew Member's Declaration). Please contact the Distribution Center via fax on (317) 290-3046 to obtain copies of individual forms in bulk. Local Port Offices should have small quantities on hand for those who require only a few copies.
Printing Serially Numbered Forms
CBP Form 5129 may be printed by individual companies as follows: Send your request on company letterhead specifying the number of forms to be printed and fax your request to the CBP Forms Manager on (202) 344-1450. You will receive a return fax with the block of serial numbers (i.e. , 10, 000cps; H0012301 - H0022300).
Printing
Forms previously printed in inks other than black may now be printed in black. Original forms with multiple components printed on color stock, must also be printed on color stock. Procedures for submitting forms have not changed.
Please ensure your data entry is correct by rechecking prior to clicking the 'Print/Print Preview' button. Be advised that you cannot edit the 'Print/Print Preview Form' and you will have to reenter all the data if corrections are required.
Format
Forms are available in three formats. Print only produces an Acrobat PDF document that can be printed and completed off-line. Online allows electronic completion and printing of the form. Wizard divides longer forms into panels or sections that can be completed one by one and then printed as one document. Final output for all formats is Acrobat PDF. Filled out forms are not able to be saved and opened later to be modified.
